Inspection and Test Plan Template for Construction: What an ITP Must Contain and Why They Get Rejected

Most inspection and test plan templates you can download are blank grids. The grid was never the problem. The problem is what goes in it, and that is exactly what gets an ITP returned on first submission. This article sets out what a construction ITP must contain, how the inspection levels work, the five reasons reviewers send them back, and how to get one approved in a single round.

What an inspection and test plan is for

An inspection and test plan (ITP) is the document that says, for one package of work, what will be checked, against which requirement, by whom, at what point, and what record will prove it. It is the link between the specification and the evidence. A principal contractor or client reviewer uses it to decide whether your controls are adequate before you start, and an auditor uses it afterwards to decide whether the records you produced are the records you promised.

ISO 9001:2015 does not use the term "inspection and test plan", but clause 8.5.1 (control of production) requires the availability of documented information defining the activities to be performed and the results to be achieved, and clause 8.6 (release of products and services) requires planned arrangements to verify that requirements have been met, with evidence of conformity and traceability to the person authorising release. The ITP is how a construction contractor satisfies both.

What a construction ITP must contain

Whatever layout your client mandates, a reviewer is looking for the same information on every line. If any of the following is missing, expect the plan back.

  1. Header block. Project, contract or package number, principal contractor, subcontractor, ITP document number, revision and date, prepared by, approved by, and a one-line statement of the scope this ITP covers. A reviewer who cannot tell from the header which drawings and specification sections the plan governs will not read further.
  2. Activity. One line per inspection or test, in construction sequence, grouped into stages. For an M&E package that is typically pre-installation (submittals, deliveries, storage), installation, inspection and testing, and handover. For civils it is more often pre-pour, pour, post-pour, or excavate, bed, lay, backfill, test.
  3. Specification reference. The clause, drawing or standard the line is checked against. "Project spec" is not a reference. "Project Specification Section 6.4; BS 7671:2018+A4:2026 Reg 526" is.
  4. Acceptance criterion. The measurable condition that means pass. This is the column most ITPs fail on, and it is dealt with in detail below.
  5. Frequency. Every item, every board, one per pour, one per batch, 10 per cent sample. The reviewer needs to know what "inspected" means in numbers.
  6. Verifying record. The document with a number that the line produces: the check sheet, test certificate, delivery ticket, calibration record or photograph log. An inspection that leaves no record did not happen.
  7. Inspection level for each party. Hold, witness, surveillance or review, set for the contractor, the principal contractor, the client or engineer, and any third party the contract names.
  8. Sign-off columns. One per party the contract requires, with name, signature and date. The columns must exist before the plan is issued, not be added when the client's inspector turns up.

Hold, witness, surveillance and review

The inspection levels are where most of the negotiation happens and where most contractors hurt their own programme. The four levels in common use are:

  • H - Hold point. Work must not proceed past this point until the named party has attended and signed. A notice period applies, typically 24 to 48 hours, set by the contract.
  • W - Witness point. The named party is invited to attend. If they do not attend within the notice period, work may proceed and the record is signed by the contractor alone, with the non-attendance noted.
  • S - Surveillance. The party may attend unannounced at any time. No notice, no stopping.
  • R - Review. Document review only. Used for certificates, calculations and test results submitted after the event.

A hold point stops work. On a distribution board installation there are perhaps two or three that matter: the technical submittal before manufacture, the pre-energisation inspection, and the test results before the board is put into service. On a concrete pour there is one that matters above all others, the pre-pour inspection, because once the concrete is placed the reinforcement can never be inspected again. An ITP with fourteen hold points tells the reviewer that nobody has thought about which ones matter, and it hands the client a programme lever they did not have before.

The five reasons ITPs get rejected

These come from the reviewing side of the desk: the reasons plans are marked up and returned, in roughly the order they occur.

1. Acceptance criteria that cannot be measured

"Installed to a good standard" is not a criterion. "Installed in accordance with the specification" is not a criterion either, because it tells the inspector nothing about what to look at. The test is simple: could two inspectors, neither of whom watched the work, reach the same verdict from what is written? If not, it is an opinion, not a criterion.

Compare a typical template line, "terminations to be completed to a satisfactory standard", with the equivalent in a worked example: "torqued to the manufacturer value using a calibrated tool and torque-marked; no exposed copper outside the terminal; BS 7671 Cl. 526." The first cannot be inspected, verified or audited. The second can.

2. No specification reference

Every acceptance criterion must point to a clause, drawing or standard the reviewer can open. If you cannot cite it, you cannot inspect against it, and a reviewer cannot approve a line they cannot check. This is also where superseded editions get caught. An ITP for electrical work that still cites BS 7671:2018+A2:2022 in late 2026 is a finding, and an easy one: Amendment 4 was published on 15 April 2026 and Amendment 3 may be used only until 15 October 2026. Confirm the current edition of every standard you cite before you issue.

3. Hold points that are not really hold points

See above. Fourteen hold points is not thoroughness, it is a failure to decide. Agree the hold and witness points with the client's engineer before you issue for approval. An ITP negotiated in advance is approved in one round. One issued cold takes three.

4. No named verifying record

Each line must produce a document with a number. A check sheet reference, a test certificate reference, a delivery ticket, a photograph log entry. "Visual inspection" in the record column means there will be no record, and the reviewer knows it.

5. Missing client or third-party sign-off columns

Check the inspection and test schedule in your contract before you issue. If the contract requires the client's representative, the building control body, the insurer's surveyor or the Temporary Works Coordinator to sign at a particular point, the column has to be there, and the party has to be named. Adding it at revision 03 means everything already signed was signed against the wrong plan.

What a worked line looks like

Here is one line from a low-voltage distribution board ITP, written the way a Tier 1 reviewer expects to receive it. The activity is the pre-energisation inspection of the completed board.

  • Specification reference: BS 7671:2018+A4:2026 Chapter 64; BS EN IEC 61439 assembly documentation; Project Specification Section 6.
  • Acceptance criterion: Schedule of inspections completed with no outstanding items; all conductors identified to Reg 514; terminals torqued to manufacturer value and torque-marked; protective conductor continuity confirmed at every outgoing way; board labelled with circuit chart and warning notices fitted; no foreign material inside the enclosure.
  • Frequency: Every board.
  • Verifying record: Schedule of inspections (Reg 642.3) and activity check sheet E-014, board serial number recorded.
  • Inspection level: Contractor H; principal contractor W; client S.

Every element of the line is measurable, cited, counted and recorded. That is the whole trick, and it applies to every line on every ITP, from earthworks to VRF.

How to complete a template for your own package

  1. Replace the header block with your project, contract and package details, and give the ITP a number that fits your project numbering convention.
  2. Delete any line that does not apply to your scope. Do not leave lines in "just in case". Every line you keep is a line you must sign.
  3. Replace every specification reference with the actual clause from your contract documents. Published standards are the starting point, but where the project specification and a British Standard differ, the project specification takes precedence.
  4. Agree the hold and witness points with the client's engineer before you issue for approval.
  5. Issue at revision 01 for approval. Never start work against an unapproved ITP. If the plan changes, everything already signed was signed against the wrong plan.
  6. Check the edition of every standard cited. Standards are amended, and the amendment dates matter.

The check sheet has to match the ITP

A common secondary finding is a site check sheet that disagrees with the approved ITP: a different tolerance, a tick box where the ITP asked for a measured value, a missing hold point. The check sheet is the record the ITP promised, so its criteria should be derived line for line from the plan the client approved. Measured values, not ticks. A nonconformity reference on the sheet, so that a failed item cannot be signed off until the NCR is closed.
