Most quality documentation fails for the same reason: it describes intent rather than evidence. We build systems around the records a reviewer, auditor or handover panel will actually ask for, then work backwards to the procedures, plans and check sheets that generate them. On rail and infrastructure schemes the Contractor's Responsible Engineer (CRE) is the named engineer accountable for a discipline's compliance with the design, the standards and the client's engineering requirements. It is a point of accountability, not a signature service. We take it on where the scope is within our competence, decline it where it is not, and say so in writing either way.
Outside rail the same function is carried out under other titles: discipline lead, technical reviewer, independent checker. The work is the same. Where a project already has a responsible engineer, we can support with verification and validation, commissioning witness and the evidence reconciliation that handover depends on.
